Under the Council’s Social Services Department, this site provides day health care services for frail elderly and disabled adults.  Services include:

 

• Rehabilitative and Restorative Therapies
• Nursing Care
• Nutritional Services
• Social Services
• Recreational Activities
• Psychiatric and Psychological Services
• Transportation

 

Services are designed to restore and maintain each participant’s optimal level of health and independence while delaying or serving as an alternative to long-term institutional placement.

 

Clients must be referred to the program by a physician and must be able to function in a day care environment.

Monday through Friday, 8:30 a.m. to 3:30 p.m.
