The Department provides individual support for Council patients and members of the community.

 

Services include:

 

Adult Day Health Care, a day care program for frail elderly and disabled adults (see link under “Services”)

 

Women, Infants and Children’s Nutritional Supplement Program, (W.I.C.) a program providing nutrition counseling and free supplemental food for pregnant women and children up to 5 years of age.

 

Information and Referral, services targeting the many non-medical issues that impact the ability of patients and members of the community to obtain the services, support and resources not offered by the Council.   

 

Assistance is offered in regard to:

 

Transportation

Employment

Emergency Shelter

Housing

Information About and Access to Benefit Programs (Medi-Cal, Medicare,      

  Social Security, General Assistance and others)

Legal Assistance

Food

Recovery Programs

Other Needs and Concerns

 

W.I.C. and Information and Referral services are available at the East and West Oakland Health Centers.

 

Service Hours

 

Monday through Friday, 8:30 a.m. To 5 p.m.
